A new vascular compression syndrome (detected on 17 October 2024):

I have come across a previously undescribed vascular compression syndrome – compression of the median cubital vein when the elbow is extended. The clinical signs are difficult or impossible venous access for injections or access placement.

The patient was a young girl with repeated hospitalizations due to a tendency to sepsis. Treatment was complicated by the inability to access the subcutaneous veins in the forearm and antecubital fossa for intravenous treatment of her infections.

A functional ultrasound revealed the cause of her particular condition.

The median cubital vein disappeared when she extended her elbow and opened to a size even larger than the brachial artery when the elbow was flexed.

Since it is common to ask the patient to extend the elbow to find a site to insert a catheter or puncture needle, it was impossible to find venous access on her arms in emergency situations.

The following images show the changing size of the median cubital vein, which collapses completely when the right elbow is stretched and dilates at the end of the video when the elbow is less stretched, while the brachial artery (CA) remains open:
